Role Overview
Cera Care Ltd is seeking a skilled Care Coordinator to manage the delivery of high-quality care services. As a key member of our team, you will be responsible for matching Care Workers with Clients based on their care needs, ensuring efficient and effective service delivery.
Key Responsibilities
- Coordinate care services to meet Client needs and preferences
- Match Care Workers with Clients based on care requirements and proximity
- Conduct care assessments and reviews to ensure high-quality service delivery
- Motivate and support Care Workers to deliver exceptional care
- Collaborate with the team to ensure seamless service delivery
Requirements
- NVQ/SVQ 2 or 3 qualification (desirable)
- Registration with the SSSC (essential)
- 2 years' experience in care coordination (desirable)
- Knowledge of CQC/CI/CIW Compliance (essential)
- Organizational and methodical approach to work
- Flexibility to adapt to changing priorities and schedules
- Excellent communication and teamwork skills
