We are seeking an experienced Rapid Transfer Discharge Coordinator to join our team, responsible for ensuring timely and safe discharges of patients requiring supportive discharge from acute settings.
The successful candidate will have a clinical background, with a minimum of 2 years' experience working at Band 6 in a health or social care environment. They will be required to work collaboratively with hospital, community, GP's, and private facilities to promote safe and robust discharge/transfer of care.
">About You
">To succeed in this role, you must possess excellent communication skills, written and oral, and be able to converse positively with all levels of staff, patients, and relatives/carers. Resourcefulness, ability to work on own initiative, and apply judgement are essential qualities for this position.
A good understanding of discharge planning, knowledge of National Policies and Acts relating to legislation to support safe and sustainable discharge, are also necessary requirements.

">We offer a competitive salary range of £40,000 - £60,000 per annum, depending on experience, plus a comprehensive benefits package including flexible working arrangements, pension scheme, and opportunities for career development.
