Community Pain Management Specialist

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Leeds Community Healthcare NHS Trust.
The successful candidate will work within the community pain management service, providing a person-centred approach to managing persistent pain. They will be responsible for undertaking face-to-face and telephone assessments with patients, producing a jointly agreed personalised pain management plan.

Main Responsibilities- Undertake comprehensive assessments and develop individualised care plans for patients with persistent pain.
- Work collaboratively with other healthcare professionals to provide holistic care and support.

We offer a competitive salary range of £30,000 - £50,000 per annum, depending on experience, as well as generous annual leave and access to NHS discount sites.
Requirements- A degree in nursing and current NMC registration.
- Previous pain management experience and strong communication skills.
