Job Overview

We are seeking a Personal Support Coordinator to work with our team in Caldicot, delivering exceptional care and support to a young client with complex health needs.

Main Responsibilities
  1. Delivering personalized care and support to clients with complex health conditions.
  2. Assisting with daily living activities, including personal hygiene, mobility, and meal preparation.
  3. Administering medication and monitoring health conditions.
  4. Providing emotional support and companionship.
  5. Maintaining accurate records of care provided and reporting any changes in clients' conditions.
  6. Collaborating with healthcare professionals and family members to ensure comprehensive care.
Compensation Package

We are offering an estimated salary of £25,000 - £30,000 per annum, dependent on experience.
