Job Summary

This is an exciting opportunity to join the Manchester-based integrated discharge team, supporting the safe and rapid discharge of service users who no longer require a hospital setting.

The Manchester Care Group has developed a multi-disciplinary hospital-based integrated discharge team to facilitate timely discharge from inpatient units across the locality and wider GMMH footprint, reducing wasted bed days in a hospital setting for Manchester locality patients.

Main Duties

The successful candidate will specialise in the facilitation of a co-occurring conditions clinical model, offering dual assessment, formulation, brief intervention, extended intervention, and planning for clients with complex dual diagnosis disorders.

They will work collaboratively with colleagues and service leaders to develop and implement dual diagnosis plans, ensuring high-quality practice within the service and embedding performance reporting into service monitoring systems.

The role will also involve developing dual diagnosis champions, group work initiatives, and policy and practice within host wards, as well as providing specialised dual diagnosis assessments and advice for referred patients.
