Job Summary

This is an exciting opportunity to support the safe and rapid discharge of service users who no longer require a hospital setting. The Manchester Care Group has developed a multi-disciplinary hospital-based integrated discharge team to facilitate timely discharge from inpatient units across the locality and wider GMMH footprint.

Main Duties of the Job

The dual diagnosis worker will specialise in the facilitation of a co-occurring conditions clinical model, offering dual assessment, formulation, brief intervention, extended intervention, and planning. This will occur either during inpatient stay or as part of the discharge plan and handover to community mental health and/or substance misuse services.

Greater Manchester Mental Health NHS Foundation Trust employs over 7,000 members of staff, delivering services from more than 160 locations. We provide inpatient and community-based mental health care for people living in Bolton, Manchester, Salford, Trafford, and Wigan, as well as specialist mental health and substance misuse services across Greater Manchester and beyond.

Job Responsibilities

Delivery of dual diagnosis case advice, in line with national best practice guidance, NICE, and Department of Health guidance, Trust policy, and in consultation with other dual diagnosis colleagues. Collaborative work with colleagues and service leaders to agree and implement dual diagnosis plans, addressing the development and sustenance of high-quality practice within the service.

Work with colleagues to implement harm minimisation measures, ensuring safe environments and minimising substance use on Trust premises. Carrying a small, specialised caseload of clients with complex dual diagnosis disorders, including clients presenting with high levels of risk and co-morbidity. Providing specialised dual diagnosis assessments and developing formulations and advice for referred clients, based on an appropriate conceptual framework of the clients' problems.
