Hospice in the Weald - Palliative Care Clinician Opportunity

We are seeking a highly motivated and experienced clinical professional to join our Hospice Outreach Service as an Associate Clinical Nurse Specialist. This is an exciting opportunity to work as part of a committed and supportive multi-disciplinary team, delivering empowering and compassionate care to individuals with a life-limiting illness.

About the Role

As an Associate Clinical Nurse Specialist, you will be responsible for providing expert clinical advice and support to patients and families in the community, including care homes and nursing homes. You will work as part of a rota of clinicians covering our seven-day service, offering telephone support, clinic, and home-based assessments.

Key Responsibilities

  • Provide expert clinical advice and support to patients and families in the community
  • Work as part of a multi-disciplinary team to deliver high-quality, patient-centered care
  • Conduct clinic and home-based assessments to identify patient needs and develop individualized care plans
  • Collaborate with other community teams and specialists to optimize patient experience and improve team knowledge and development

Requirements

To be successful in this role, you will need to be a motivated clinician with advanced communication skills and excellent presentation skills. You will also need to be able to work independently as part of a responsive and resilient team.
