About the Role

We are seeking a motivated and experienced clinical professional to join our Hospice Outreach Service as an Associate Clinical Nurse Specialist. This is an exciting opportunity to develop your skills and expertise in palliative care, working as part of a multidisciplinary team to deliver high-quality care to patients and their families in the community.

Key Responsibilities

  • Provide expert clinical advice and support to patients and families in the community, including care homes and nursing homes.
  • Conduct clinic and home-based assessments to identify the needs of patients and develop individualized care plans.
  • Work collaboratively with other healthcare professionals to optimize patient care and outcomes.
  • Develop and maintain relationships with community teams and specialists to ensure seamless care coordination.

Requirements

  • Registered nurse or paramedic with experience in palliative care or a related field.
  • Advanced communication and presentation skills, with the ability to work independently and as part of a team.
  • Ability to work flexibly, including evenings and weekends, to meet the needs of patients and the service.
