Job Summary

We are seeking a highly motivated and experienced Clinical Nurse Specialist to join our Hospice Outreach Service team. As a key member of our multidisciplinary team, you will provide high-quality, patient-centered care to individuals with life-limiting illnesses and their families.

About the Role

This is an exciting opportunity for a clinical professional to develop their skills and expertise in palliative and end-of-life care. You will work closely with our team to provide telephone support, clinic and home-based assessments, and palliative advice and support to patients and families in the community.

Key Responsibilities
  • Provide high-quality, patient-centered care to individuals with life-limiting illnesses and their families.
  • Work closely with our team to provide telephone support, clinic and home-based assessments, and palliative advice and support to patients and families in the community.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in multidisciplinary team meetings and contribute to the development of care plans.
  • Stay up-to-date with best practices and evidence-based guidelines in palliative and end-of-life care.
Requirements

To be successful in this role, you will need:

  • A degree in nursing or a related field.
  • Experience in palliative and end-of-life care.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work a variety of shifts, including evenings and weekends.
