About the Role

Hospice in the Weald is seeking a highly motivated and experienced Clinical Nurse Specialist to join our Hospice Outreach Service team. As a key member of our multi-disciplinary team, you will play a vital role in delivering high-quality, patient-centered care to individuals with life-limiting illnesses in the community.

Key Responsibilities

  • Provide expert clinical advice and support to patients and families in their homes, care homes, and nursing homes.
  • Conduct thorough assessments and develop individualized care plans to improve quality of life and achieve patient-centered goals.
  • Work collaboratively with other community teams and specialists to optimize patient experience and improve team knowledge and development.
  • Participate in the development and implementation of palliative care services in the community.

Requirements

  • Advanced practice experience in community or in-patient palliative care at Band 6 or 7 level or equivalent.
  • Proven ability to work as an independent practitioner within a responsive and resilient team.
  • Excellent communication and presentation skills, both oral and written.
  • Ability to work flexibly and adapt to changing priorities.
