About the Role:
As a skilled RN Case Manager, you will be responsible for conducting comprehensive assessments, developing and implementing individualized care plans, and coordinating with healthcare professionals to ensure seamless transitions of care. Your expertise will enable you to provide guidance and support to caregivers, families, and other stakeholders, empowering them to navigate the complexities of home health care.

Key Responsibilities:
* Conduct thorough initial and reassessment visits, incorporating input from clients and their loved ones
* Develop, implement, and evaluate individualized care plans tailored to each client's unique needs and goals
* Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care
* Provide education, coaching, and monitoring to clients and caregivers on disease management, medication administration, and other aspects of care
* Maintain accurate and timely documentation, adhering to agency policies and regulatory requirements

Requirements:
* Current licensure as a Registered Nurse (RN) in the state of employment
* Minimum one year of experience in acute care or home health nursing
* Excellent communication, organizational, and problem-solving skills
* Ability to work independently and collaboratively within a dynamic team environment
* Strong clinical knowledge and judgment, with the ability to prioritize tasks and manage multiple responsibilities
