An exciting opportunity has arisen within South Tees Hospitals NHS Foundation Trust to join our Urgent Community Response (UCR) and Hospital at Home service. We are seeking skilled and outstanding senior clinicians to provide high-quality care to patients in their own homes.
Main ResponsibilitiesAs a Community Clinical Nurse Practitioner, you will be a key member of our team, providing in-depth assessments of nursing care and treatment for patients in their own homes. You will work closely with other healthcare professionals to prevent unnecessary admissions and facilitate timely discharge from hospital.
Key responsibilities include:
- Conducting comprehensive assessments of patients' needs and developing individualized care plans
- Providing high-quality nursing care and treatment to patients in their own homes
- Working collaboratively with other healthcare professionals to ensure seamless care transitions
- Participating in regular clinical supervision and peer review
To be successful in this role, you will need:
- To be an independent non-medical prescriber with demonstrable experience
- To have completed the advanced physical assessment and disease management skills course (level 7) or have completed the physical assessment and disease management skills course (level 6) and be willing to work towards the level 7 course within the first 2 years of appointment
- To hold a Diploma or Degree awarded by a professional body
- To be registered as fit to practice by the Nursing and Midwifery Council (NMC) as a registered adult nurse
- To have significant post-registration experience and advanced clinical skills
