Job Overview

We are seeking a skilled Clinical Care Specialist to join our Community Nursing team in Manchester. As a key member of the team, you will work closely with Registered Nurses and other healthcare professionals to provide high-quality care to patients in their homes and community settings.

Your primary responsibility will be to assess patient needs, develop and implement individualized care plans, and deliver evidence-based interventions to promote optimal health outcomes.

We offer a competitive salary of £35,000 - £40,000 per annum, depending on experience, as well as opportunities for career development and professional growth within the trust.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Deliver evidence-based interventions to promote optimal health outcomes
  • Collaborate with multidisciplinary teams to coordinate care
  • Monitor patient progress and adjust care plans as needed

Requirements:

  • NMC Registered Nursing Associate qualification
  • Ability to work autonomously and as part of a team
  • Excellent communication and interpersonal skills
  • Experience working in community settings (desirable)
