About the Role

We are seeking a dedicated and compassionate Clinical Care Specialist to join our team at Manchester University NHS Foundation Trust (MFT). As a key member of our Discharge to Assess team, you will play a vital role in providing high-quality patient-centered care to adults in the community in South Manchester.

Your primary responsibility will be to work under the supervision of Registered Nurses and within Trust guidelines & protocols to deliver exceptional patient care. This will involve supporting patients with personal care needs, traveling to patients' homes from various locations, and recording patient observations & changes to clinical conditions.

In this role, you will have the opportunity to develop your skills and knowledge through on-the-job training & assessment. You will learn how to undertake a range of delegated duties, including recording patient observations & changes to clinical conditions.

To be successful in this position, you will require a car or access to a reliable mode of transportation. Previous community experience is preferred, but training will be available for the right candidate.

As a Clinical Care Specialist, you will be part of a multi-disciplinary health & social care team working towards regaining or maintaining independence and quality of life for our patients.
