**Job Title:** Care Team Leader

**Job Summary:**

We are seeking a highly experienced and dedicated Care Team Leader to join our team at Snap Care. As a Care Team Leader, you will be responsible for managing a busy and dedicated team of support workers who provide day and waking night support to a 13-year-old girl with complex care needs.

**Key Responsibilities:**

* Manage a team of support workers to provide high-quality care to Georgie
* Develop and implement care plans to meet Georgie's individual needs
* Coordinate with Georgie's multidisciplinary team to ensure seamless care delivery
* Supervise and train support workers to ensure they have the necessary skills and knowledge to provide excellent care
* Liaise with Georgie's family to ensure their needs are met and their expectations are exceeded

**Requirements:**

* 1 year minimum experience in a team leader/senior/managerial role within health and social care
* Paediatric support or childcare experience
* Ability to work independently and as part of a team
* Excellent communication and interpersonal skills
* Ability to manage a team and prioritize tasks effectively
