Position Overview

The Urgent Community Response Team (UCR) is dedicated to delivering a responsive, safe, and compassionate service that is effectively managed. The primary objectives of the service include urgent case management, recovery assistance, self-management, and fostering a collaborative approach with patients. Operating from local hubs, UCR provides a cohesive strategy across a designated area, ensuring excellence in care while engaging with all GP practices, community teams, and broader stakeholders.

The role of the specialist occupational therapist is a dynamic opportunity to provide care to individuals at risk of hospital admission within their residences. This position also involves participation in the award-winning collaborative Falls Car initiative, developed in partnership with emergency medical services.

The service operates throughout the week, offering flexible working arrangements. The role requires the completion of one shift per week alongside a specialist paramedic, focusing on the Falls Car initiative. There are also opportunities for collaboration with local acute trusts through established partnerships.

Key Responsibilities

- Collaborate closely with Consultant Practitioners, Advanced Clinical Practitioners (ACPs), the Allied Health Professional Lead, Team Lead, and the broader nursing and therapy teams, as well as GPs, to provide prompt and effective services for patients experiencing exacerbations of chronic conditions and/or frailty. This includes ensuring rapid treatment delivery, care planning, and admission avoidance when appropriate.

- Conduct assessments and develop care plans, including comprehensive history taking, physical evaluations, and treatment strategies, with minimal supervision.

- Work in partnership with the Allied Health Professional Lead, Team Lead, Consultant Practitioners, ACPs, GPs, and patients to proactively identify and manage individuals with acute health needs, providing support to both patients and their caregivers. Delegate suitable tasks to nursing and therapy colleagues, Associate Practitioners, and Health Care Support Workers, while offering clinical supervision to enhance their professional development.

Qualifications

Essential

  • Qualified Occupational Therapist with HCPC registration.

Desirable

  • Postgraduate qualification.

Experience

Essential

  • Relevant post-registration experience across various settings.
