We are seeking a dedicated and compassionate Care and Support Coordinator to join our team in Sevenoaks, Kent. As a Care and Support Coordinator, you will play a vital role in providing exceptional care and support to learning disabled adults, empowering them to live their best lives.

Whether it's supporting daily tasks, planning social activities, or advocating for our service users' rights, you will be instrumental in helping them thrive in their community.

Key Responsibilities
  • Develop and implement personalized care plans, taking into account the unique needs and aspirations of our service users
  • Provide high-quality care and support, promoting independence, confidence, and well-being
  • Build strong relationships with service users, families, and colleagues to ensure seamless delivery of care services
  • Collaborate with healthcare professionals, local authorities, and other stakeholders to advocate for our service users' needs
  • Contribute to ongoing training and development, staying up-to-date with best practices and industry developments

Requirements

To be successful in this role, you will need:

  • Relevant experience in care and support, preferably within a similar setting
  • An excellent understanding of the needs and aspirations of learning disabled adults
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals
  • A flexible approach, with a willingness to adapt to changing circumstances
  • Enhanced DBS clearance and any other relevant certifications
