Senior Community Mental Health Practitioner

Key Responsibilities- Provide high-standard clinical care, ensuring safe and effective assessment, treatment, and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.
- Delegate aspects of the patient treatment plan to other team members, while maintaining overall responsibility for patients' care, where appropriate.
- Ensure the clinical caseload and clinical practice of nursing members of the Community Mental Health Team is of the highest standard of clinical care.
- Work with individuals to assess their mental health, recognize mental illness, and identify their related needs and circumstances.
- Plan, implement, review, and improve interventions to meet people's identified needs and manage their inherent risk.
- Enable individuals to develop independent living skills and live in the community.
We are seeking a dynamic and compassionate individual to join our team as a Senior Community Mental Health Practitioner - Adult Mental Health. The successful candidate will have experience and understanding of an array of person-centred, holistic, recovery-focused interventions and will be able to demonstrate a dynamic, creative, and positive approach.
