Personal Independence Coordinator

We are seeking a compassionate and person-centred individual to join our team as a Personal Independence Coordinator. As a key member of our multidisciplinary team, you will work alongside health and care professionals to provide critical links between formal health and social care services and the wider community support networks.

About the Role

In this role, you will have the opportunity to make a positive difference in the lives of older people in Croydon. You will work collaboratively with our team to provide support and enablement to individuals with long-term health and social care needs.

Key Responsibilities
  • Provide person-centred support and enablement to individuals with long-term health and social care needs
  • Work collaboratively with health and care professionals to provide critical links between formal health and social care services and the wider community support networks
  • Develop and maintain relationships with community groups and organisations to enhance the support and services available to our clients
  • Contribute to the development and implementation of our service delivery model
