Job Overview

Sonderwell, a leading provider of high-quality home care services, is seeking an experienced Field Care Manager Lead to oversee the delivery of exceptional care to clients in Gloucestershire and South Wales.

About the Role
  • This is a remote/field-based position that requires regular travel to client homes, with some flexibility over weekends and evenings.

The successful candidate will be responsible for conducting client assessments, managing care plans, and ensuring compliance with CQC regulations.

Key Responsibilities:

  • Develop and review personalized care plans to meet the unique needs of each client.
  • Liaise with the operational team to address performance issues and training needs.
  • Ensure services meet CQC standards by conducting audits and maintaining accurate records.
  • Build strong relationships with clients and their families, addressing concerns and updating care needs as required.
  • Collaborate with healthcare professionals to deliver holistic care solutions.

Requirements:

  • NVQ Level 3 or 4 in Health and Social Care (or equivalent).
  • Experience in a supervisory/management role within health and social care.
  • Excellent communication and organizational skills, with a passion for delivering high-quality care.
  • A valid UK driving license and ability to travel using own car.
