Vulnerable Adults Care Coordinator

We are seeking a Vulnerable Adults Care Coordinator to join our team in Croydon. As a key member of our team, you will be responsible for coordinating the delivery of support services to our clients, ensuring that they receive the highest level of care and support.
Key Responsibilities- Coordinate the delivery of support services to clients
- Assess client needs and develop support plans
- Work in partnership with external agencies to provide comprehensive support services
- Monitor and evaluate the effectiveness of support services
- Experience of working with vulnerable adults
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Good knowledge of welfare benefits and housing options
£30,000 - £35,000 per annum, depending on experience. We offer a competitive salary, generous holiday entitlement, and opportunities for career development.
