Job Summary

We are seeking a highly skilled and experienced Physiotherapist or Occupational Therapist to join our Urgent Community Response (UCR) team. As a key member of the multi-disciplinary team, you will be responsible for undertaking assessments, clinical diagnosis, and treatment of patients, including telephone triage, multi-factorial falls assessments, and patients who may require crisis management and/or rehabilitation interventions to prevent acute hospital admissions and to support complex hospital discharges.

About the Role

You will work closely with the Physiotherapy Team Lead and other multidisciplinary team members to ensure timely and effective communication on all professional matters. You will also be responsible for maintaining accurate documentation in line with the Chartered Society of Physiotherapy standards and CSH Surrey standards and policies.

Key Responsibilities
  • To communicate complex and sensitive information to patients, carers, and other staff, including imparting unwelcome news relating to their rehabilitation potential and gaining consent for treatment.
  • To ensure timely and effective communication with the Physiotherapy Team Lead and other multidisciplinary team members on all professional matters.
  • To maintain accurate documentation in line with the Chartered Society of Physiotherapy standards and CSH Surrey standards and policies.
  • To use a range of verbal and non-verbal communication skills to communicate effectively with patients in order to progress rehabilitation and treatment programmes.
  • To appreciate cultural differences and take these into account when agreeing treatment plans with patients.
  • To undertake comprehensive assessments of patients, including those with diverse or complex presentations or multi-pathologies.
  • To participate in the supervision of students and deputise for the Physiotherapy Team Lead when required.
  • To provide training and take a role in supervision and teaching of qualified and unqualified clinical staff, including nurses, occupational therapists, and rehabilitation assistants.
  • To ensure professional competence is maintained by participating in all Continuing Professional Development (CPD) opportunities and maintain a portfolio of evidence.
Person Specification
  • Registration with the relevant Registration Body if applicable.
  • Degree or Equivalent in Physiotherapy or Occupational Therapy.
  • Minimum of 2 years post-qualification & NHS experience.
  • Knowledge of a wide range of client groups, including community, rehabilitation, hospital, neurology, elderly, oncology, and orthopaedics.
