We are seeking a highly skilled and experienced Physiotherapist/Occupational Therapist to join our team as a Clinical Specialist Therapist for Older Adults. This is an exciting opportunity to work in a dynamic and supportive environment, contributing to the development of a new discharge to assess model within Cheshire Place.
Key Responsibilities- Coordinate and facilitate the safe and effective discharge of patients from Macclesfield District General Hospital
- Provide initial assessments and interventions in the home environment where indicated
- Work proactively within an integrated health and social care team to promote formal and informal channels of communication
- Deliver clinical therapy assessment and interventions for patients across the acute and community settings within East Cheshire NHS Trust

Requirements- HCPC registration
- Relevant professional degree/diploma
