Job Summary
We are looking for a highly skilled and enthusiastic Senior Occupational Therapist to join our Integrated Community Stroke Rehabilitation Service in East Cheshire. As a key member of our team, you will be responsible for providing early rehabilitation for stroke patients, setting goals and treatment plans, and promoting a seamless transition from the acute hospital to the home environment.

Key Responsibilities:
• Provide high-quality occupational therapy services to stroke patients in their own homes or normal place of residence.
• Develop and implement individualized treatment plans to meet the unique needs of each patient.
• Work closely with the interdisciplinary team to ensure a comprehensive and coordinated approach to patient care.
• Maintain accurate records and communicate effectively with patients, families, and healthcare professionals.
