Job Summary
We are seeking a skilled and compassionate Mental Health Specialist to join our team at South West Yorkshire Partnership NHS Trust. As a key member of our community care team, you will be responsible for providing high-quality interventions to individuals with mental health problems in the community.
Key Responsibilities
- Develop and implement effective care plans in partnership with individuals and their carers.
- Provide skilled interventions to promote health and well-being, and prevent illness.
- Establish and maintain effective communication with all agencies involved in an individual's care.
- Maintain accurate patient care records and participate in the formulation of policies at unit level.
Requirements
- Professional registration and commitment to statutory codes of practice and continued professional development.
- Ability to work effectively in a team and communicate with individuals and carers.
