Care Team Lead

- Conduct regular home visits with foster carers and children/young people to monitor their well-being and provide support as needed.
- Develop and implement care plans to meet the unique needs of each child or young person.
- Participate in an on-call rota to provide 24/7 telephone support to carers.
- A recognised social work qualification (e.g., BSW, MSW).
- Current registration with Social Care Wales.
- A full UK driving licence and vehicle.
- An enhanced DBS check.
- Experience in children's services and foster care delivery.
- Ability to participate in out-of-hours telephone support.
