Care Team Responsibilities
  • Conduct regular home visits with foster carers and children/young people to monitor their well-being and provide support as needed.
  • Develop and implement care plans to meet the unique needs of each child or young person.
  • Participate in an on-call rota to provide 24/7 telephone support to carers.
Requirements
  • A recognised social work qualification (e.g., BSW, MSW).
  • Current registration with Social Care Wales.
  • A full UK driving licence and vehicle.
  • An enhanced DBS check.
  • Experience in children's services and foster care delivery.
  • Ability to participate in out-of-hours telephone support.
