We are seeking a motivated and compassionate Rehabilitation Assistant to join our Home First & Reablement service in Cheltenham. As a key member of our team, you will play a vital role in supporting individuals to regain their independence and live safely in their own homes.
Key Responsibilities- Provide practical, personal, and intimate care to service users, maintaining high standards of hygiene and health and safety.
- Assist service users in achieving their outcome-focused goals, enabling them to relearn or develop new skills and abilities required for daily living.
- Monitor progress and function of service users within their reablement programme, contributing to the ongoing assessment of their rehabilitation needs.
- Identify and escalate safeguarding alerts, ensuring timely reporting of suspected abuse and self-neglect.
- Support service users and their families/carers in understanding their Reablement plan, including the correct use of equipment.
- Adhere to health and safety instructions, reporting incidents, emergencies, or accidents to the line manager.
- Connect service users to community resources, promoting digital inclusion and the use of assisted technology telecare.
- GCSE level Pass or equivalent in Maths and English.
- Care Certificate.
- Desirable: NVQ in a relevant topic (such as healthcare) or other relevant training.
