About the Role

As a Clinical Pharmacist, you will play a key part in delivering specialist clinical pharmacy services within care homes in Hexham and the surrounding areas. You will work closely with the multi-disciplinary team to provide person-centred, evidence-based structured medication reviews for care home residents and frail elderly patients in their own homes.

The role requires you to have a strong clinical background, with experience of working with primary care colleagues and managing staff. You will also be responsible for promoting safe and effective use of medicines, minimising waste, and reducing medicines-related incidents/errors.

This is an exciting opportunity to join our team and contribute to the high-quality patient care that we strive to deliver. The salary for this role is $90,000 - $110,000 per annum, depending on experience.

Key Responsibilities
  1. To support the clinical pharmacy team in delivering specialist clinical pharmacy services within care homes in Hexham and the surrounding areas.
  2. To complete targeted reviews of care home residents and frail elderly patients to help minimise risk at transfer of care.
  3. To undertake risk management and clinical governance activities within your assigned areas of responsibility.
Person Specification
  • Previous pre-registration training and registration with the General Pharmaceutical Council (GPhC).
  • A postgraduate clinical qualification and/or significant relevant clinical experience.
  • Evidence of continuing professional development to meet the mandatory requirements of the GPhC.
