About Us:

Your New Role:

We are seeking an experienced Advanced Nurse Practitioner to join our dynamic Community Treatment Team (CTT) that covers Barking & Dagenham, Havering, and Redbridge. As a key member of the team, you will play a vital role in managing patients in crisis in their own homes, avoiding unnecessary admissions to acute hospital settings.

Key Responsibilities:

  • Deliver high-quality holistic nursing care as part of a multidisciplinary team within the accident and emergency and community setting.
  • Undertake comprehensive health assessments, physical examinations, and ordering tests for patients in Rapid Response.
  • Use advanced clinical decision-making skills to coordinate care planning and initiate referrals to diagnostics for appropriate tests.
  • Play a leadership role within the team and work with external stakeholders to develop new ways of working and increase referrals from acute and community settings.

Requirements:

  • Qualified Advanced Nurse Practitioner
  • Registration with a health governing body (e.g., NMC)
  • Mentor/teaching qualifications
  • Clinical supervision training and experience
  • Extended/independent prescribing

Salary: £55,000 - £65,000 per annum
