Job Title: Older Adults Community Nursing Position

Company Overview: Norfolk & Suffolk Foundation NHS Trust is a respected healthcare provider, dedicated to delivering person-centered care to our community. We are seeking an experienced Community Nurse to join our Older People's Team.

Salary: £38,000 - £48,000 per annum (dependent on experience)

Job Description:
In this rewarding role, you will work alongside a dedicated multidisciplinary team to provide high-quality care to older adults. Your responsibilities will include managing a caseload, assessing and planning treatment, and collaborating with patients, carers, and other healthcare professionals to ensure optimal care delivery. As a key member of the team, you will participate in the Duty Rota and contribute to the development of individualized treatment plans.

Required Skills and Qualifications: A nursing degree or equivalent, Dip OT, Clinical Psychology, or equivalent. You must also be able to travel independently.
