Mental Health Liaison Nurse

Oxleas NHS Foundation Trust is seeking an experienced mental health nurse to join our Older Adults Mental Health Liaison Team. As a Mental Health Liaison Nurse, you will provide high-quality care and support to patients in the emergency department and on inpatient wards.
The ideal candidate will have a strong understanding of older peoples' mental health issues, with experience in assessing and managing risk. You will be skilled in biopsychosocial assessments, effective communication, and working collaboratively within a multi-disciplinary team.
This is an excellent opportunity to work in a dynamic environment, providing comprehensive care to individuals referred to the team. Your expertise will enable you to develop interventions suitable for this client group, while also contributing to the team's creative and supportive approach.
Responsibilities
- To conduct timely and effective biopsychosocial assessments of individuals referred to the team.
- To consider all necessary sources of information in formulating the assessment and care plan.
- To work as part of a multi-disciplinary team in carrying out effective risk assessment and management.
