About the Role

We are seeking a motivated and dynamic individual to join our Pathfinder Team as an Assistant Practitioner/Nursing Associate. This is an exciting opportunity to work in a specialist community team based within North Devon District Hospital, providing holistic assessments and onward care for specific patient groups.

Key Responsibilities
  • Assess and procure onward care for patients with complex needs
  • Work closely with therapy, nursing, and paramedic teams to facilitate patient flow through the acute hospital
  • Provide follow-up assessments and support in the community to ensure safe discharge
  • Assist with basic administration tasks to support the daily running of the team
Requirements

We are looking for a forward-thinking individual with excellent communication skills and the ability to work under pressure. Experience from acute and community backgrounds is preferred, but not essential. Training and support will be provided to the successful applicant.
