About the Role

We are seeking a motivated and dynamic individual to join our Pathfinder Team as a Assistant Practitioner/Nursing Associate. The successful candidate will work closely with our community colleagues to provide seamless transition of care between hospital and home or other receiving destinations.

Key Responsibilities
  • Assess and procure onward care for specific patient groups, including admission avoidance, short stay admissions, frailty acute hospital at home, and end of life care discharges.
  • Work in collaboration with therapy, nursing, and paramedic backgrounds to provide holistic care.
  • Develop and maintain relationships with community teams to ensure effective discharge planning.
  • Provide education and support to patients and their families to promote independence and self-care.
  • Contribute to the development of a Frailty pathway and participate in initiatives around length of stay and discharge planning.
Requirements
  • Experience in acute and community settings, preferably in a healthcare or social care role.
  • Ability to work under pressure and as part of a team.
  • Strong communication and interpersonal skills.
  • Basic IT skills, with ability to learn and adapt to new systems.
