About the Role

We are seeking a motivated and dynamic individual to join our Pathfinder Team as a Assistant Practitioner/Nursing Associate. The successful candidate will work closely with our community colleagues to provide seamless transition of care between hospital and home or other receiving destination.

Key Responsibilities
  • Assess and procure onward care for specific patient groups, including admission avoidance, short stay admissions, frailty acute hospital at home, and end of life care discharges.
  • Work in collaboration with therapy, nursing, and paramedic backgrounds to provide holistic care.
  • Develop and implement care plans to support patients in their recovery and rehabilitation.
  • Collaborate with community teams to ensure a smooth transition of care.
  • Provide education and support to patients and their families to promote self-management and independence.
Requirements
  • Experience in acute and community settings, preferably in a healthcare or social care role.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work under pressure and prioritize tasks to meet deadlines.
  • Basic IT skills, with the ability to learn and adapt to new systems and technologies.
