We are seeking an experienced and dynamic District Nurse to lead our team in providing high-quality, patient-centered care to the community. As a key member of our team, you will be responsible for managing a multidisciplinary team, providing leadership and guidance, and ensuring the delivery of safe and effective care to patients.
Key Responsibilities- Provide leadership and management to a multidisciplinary Community Nursing team, ensuring the provision of high-quality, effective, well-coordinated, and responsive care to the changing needs of patients and the local population.
- Demonstrate effective leadership by taking an active role in the professional development of the workforce, improving standards, and promoting good practice and clinical effectiveness.
- Work in an integrated way with partner agencies to ensure the highest quality delivery of care, facilitating the putting of the patient at the centre of care, avoiding duplication of assessments and treatment, and managing long-term conditions in a proactive way to avoid crisis and unnecessary hospital admission.
We are looking for a candidate with:
- RGN / ECP / Paramedic (HCPC) with current registration.
- Specialist postgraduate education to degree level or equivalent in a relevant specialist area.
- Community Specialist practice teacher qualification.
Experience of working with a multi-disciplinary Primary Health Care Team, significant post-registration experience working in the community setting, and evidence of involvement in project development work or health promotion group work are essential. A DN community experience is desirable.
