Advanced Community Nurse Practitioner

We are seeking a highly skilled and experienced Lead Nurse to join our Integrated District Nursing Team in Cambridgeshire Community Services NHS Trust.
This is an exciting opportunity for a dynamic and strategic individual to lead a multidisciplinary team in delivering high-quality, patient-centered care to the local community.
The successful candidate will have a strong background in community nursing, with experience in managing teams and promoting good practice and clinical effectiveness.
Key responsibilities will include providing leadership and management to the team, working in partnership with other agencies to ensure seamless care, and promoting a positive image of community nursing.
The ideal candidate will have a systematic approach to work, be able to prioritize and manage time effectively, and demonstrate empathy and strong communication skills.

Person Specification- RGN / ECP / Paramedic (HCPC) with current registration
- Specialist postgraduate education to degree level or equivalent in a relevant specialist area
- Community Specialist Practice Teacher qualification
- Experience of working with a multidisciplinary Primary Health Care Team
- Significant post-registration experience working in the community setting
- Understanding and experience of staff and resource management
- Able to demonstrate experience and knowledge of clinical / governance and audit
- Evidence of involvement in project development work or health promotion group work
- Experience of managing patients with Acute/Chronic long-term conditions
- DN community experience
- Experience of working within a diverse population
