GP Pharmacist

We are seeking an experienced General Practice Pharmacist to join our team as a Clinical Leader. This is an exciting opportunity to work in a dynamic and innovative environment, integrating the clinical skills of a pharmacist into the general practice team.
The successful candidate will be responsible for providing clinical pharmacy leadership for the practice, taking responsibility for Structured Medication Reviews, chronic disease management, medicines optimisation, quality improvement, and other enhanced services. They will work as part of a multi-disciplinary team, providing clinical expertise and supporting practice staff with prescription and medication queries.
Key Responsibilities:- Provide clinical pharmacy leadership for the practice
- Develop and implement clinical pharmacy services
- Work with practice staff to improve patient care and outcomes
- Provide education and training to practice staff
- Contribute to quality improvement initiatives
- Minimum of 5 years post-registration experience with at least 2 years in General Practice
- Certificate in Non-medical prescribing
- Experience of using prescribing qualification in General Practice
- Evidence of further education in clinical pharmacy
