About the Role

We are seeking a highly motivated and organized Care Coordinator to join our team at Purbeck Primary Care Network. As a Care Coordinator, you will play a key role in supporting our digital services and working closely with our Enhanced Health in Care Homes Team.

Main Responsibilities

  • Provide administrative support to our digital services, including managing patient lists, coordinating appointments, and ensuring seamless communication between teams.
  • Work closely with our Enhanced Health in Care Homes Team to manage patient lists, coordinate visits, and ensure timely follow-up.
  • Assist with the implementation of digital projects across the network, including the rollout of new technologies and education of patients.
  • Support the development of social media content for the network, including website and Facebook updates.
  • Provide data administration support to the network leadership team and member practices as required.

Requirements

  • Good understanding of health and social care.
  • Digitally literate with good working knowledge of digital systems and processes.
  • Proven experience of working with the general public in a similar role.
  • Experience of working in a service provision and/or healthcare organization.
