Community Care Coordinator

We are seeking a dedicated Community Care Coordinator to support individuals with complex needs, delivering outstanding care and leading a team with compassion and professionalism.

About the Role:
  • This is an excellent opportunity for a compassionate and skilled individual to join our team as a Community Care Coordinator.
  • You will be responsible for managing staff rostering, supporting care workers in managing challenging behaviour, and working closely with healthcare professionals and family members to ensure holistic, person-centred care.
Key Responsibilities:
  1. Be the main point of contact for Clients and Care Workers, ensuring excellent communication and care.
  2. Manage staff rostering to ensure care is delivered safely and effectively to those with learning disabilities and challenging behaviour.
  3. Support care workers in managing challenging behaviour, helping to create care plans that address individual needs and promote positive outcomes.
  4. Work closely with healthcare professionals and family members to ensure holistic, person-centred care.
  5. Supervise, motivate, and appraise care workers, supporting them in handling challenging situations with confidence and compassion.
  6. Handle safeguarding issues, incidents, and complaints, always focusing on delivering solutions and support.
  7. Ensure services meet and exceed CQC standards.
Benefits:

The salary for this role is £25,000 per annum, plus benefits including medicash healthcare benefits, prescription refunds, dental care, pension scheme, and more.
