Job Description

We are seeking a highly skilled and compassionate Field Care Supervisor to join our team at Altogether Care. As a Field Care Supervisor, you will play a vital role in ensuring the delivery of high-quality care services to our clients.

**Key Responsibilities:**

  • Assess care needs and develop personalized care plans
  • Monitor and review care plans to ensure they meet the evolving needs of our clients
  • Supervise and mentor care colleagues to ensure they have the necessary skills and knowledge to provide exceptional care
  • Conduct spot checks to ensure compliance with care standards and regulations
  • Support the on-call service on a rotational basis to provide 24/7 care coverage
  • Provide coaching and mentoring to care colleagues to enhance their skills and knowledge
  • Attend care reviews to ensure that care plans are up-to-date and meet the needs of our clients

**Requirements:**

  • Proven experience in a care leadership role
  • Strong knowledge of care standards and regulations
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work a variety of shifts, including evenings and weekends
