Care Team Leader

We are seeking a compassionate and caring individual to join our team as a Field Care Supervisor. As a Field Care Supervisor, you will be responsible for delivering high-quality care and support to our clients in their own homes.
Key Responsibilities:
- Write and update new and existing care plans
- Coach and mentor care colleagues
- Attend care reviews
- Support the on-call service on a rota basis
- Provide "hands on" care and support
- Build and maintain positive relationships with our clients, family members, colleagues, and other professionals
- Utilise technology to deliver quality care
- Uphold the principles of Altogether Care's values
- Ensure the implementation of safe working practices

About the Role
As a Field Care Supervisor, you will be responsible for:
· Delivering high-quality care and support to our clients
· Building and maintaining positive relationships with our clients, family members, colleagues, and other professionals
· Utilising technology to deliver quality care

· Ensuring the implementation of safe working practices
