Job Title: Support Worker

We are seeking a dedicated and compassionate Support Worker to join our team in the West Kent locality. As a Support Worker, you will play a vital role in supporting individuals with learning disabilities to access and participate in their local community.

Key Responsibilities:
  • Provide person-centred support to individuals with learning disabilities, promoting their social inclusion and rights.
  • Work collaboratively with colleagues to develop and implement individualised support plans.
  • Assist individuals with daily living tasks, such as personal care and medication management.
  • Support individuals to access community resources and activities, including education and employment opportunities.
  • Work in a variety of settings, including community hubs and individual homes.
Requirements:
  • Diploma Level 2 in Health and Social Care or equivalent.
  • Experience of working with individuals with learning disabilities, preferably in a community setting.
  • Ability to work flexibly, including evenings and weekends.
  • Enhanced Disclosure and Barring Service (DBS) clearance.
