We are seeking a skilled Mental Health Practitioner/Specialist Nurse to join our team at Midlands Partnership University NHS Foundation Trust. As a key member of our team, you will provide high-quality care and support to patients in their own homes, helping them to manage their personal care, prepare meals, and safely take their medication.

Key Responsibilities- Provide personalized care and support to patients in their own homes
- Assist patients with managing their personal care, preparing meals, and safely taking medication
- Work closely with our multidisciplinary team to ensure the best possible outcomes for patients
- Develop and implement personalized care plans to meet the unique needs of each patient
- Monitor patient progress and adjust care plans as needed
