Key Responsibilities:
- Provide individualized support to adults with learning and physical disabilities, promoting their independence and well-being.
- Work collaboratively with individuals, their families, and other healthcare professionals to develop and implement personalized support plans.
- Assist individuals in achieving their personal outcomes, such as maintaining a safe and healthy lifestyle, engaging in meaningful activities, and building relationships.
- Contribute to a person-centered, outcome-focused approach, working in partnership with Renfrewshire Council Social Work Department and community healthcare teams.
- Participate in staff training and development opportunities to enhance your skills and knowledge.
Requirements:
- IT skills are required for this role.
- Successful candidates will be required to register with the SSSC within 6 months of start date.
