About the Role

As a Community Healthcare Professional, you will be responsible for providing high-quality care and support to patients in our community hospital wards. You will work collaboratively with other healthcare professionals to develop individualized care plans and provide holistic support to patients with complex needs.

Key Responsibilities
  • Conduct comprehensive assessments of patients to identify their unique care needs.
  • Develop and implement individualized care plans in collaboration with patients and their families.
  • Work collaboratively with multidisciplinary teams to ensure seamless care transitions.
