Job Summary

We are seeking an experienced nurse to join our team as a Clinical Commissioning Case Manager for Continuing Healthcare. The successful candidate will work across the Birmingham and Solihull area, supporting individuals who are eligible for NHS Continuing Healthcare. The role requires excellent time management, effective communication skills, and the ability to work collaboratively with a range of health and care professionals.

The post holder will be responsible for attending Multi-Disciplinary Meetings, Acute Care Facilities, individuals' homes, and a variety of care provider settings. They will also be required to regularly review individuals in line with The National Framework for NHS Continuing Healthcare, case management requirements. Additionally, they will be responsible for negotiating care costs and following the agreed ICB processes for agreement of care packages.

We are a clinically led service, and we will offer support to develop your skills and knowledge in this very skilled area of health provision. This role offers an opportunity to support some of the most vulnerable and complex individuals who live in our community, improving their outcomes and quality of life.

About the Role

The successful candidate will have previous experience of undertaking all age Continuing Care Assessments, experience of Complex Case Management, and a good understanding of Safeguarding issues and policy framework. They will also have a good understanding and/or experience of CHC National Framework.
