All Age Continuing Healthcare Clinical Commissioning Case Manager

We are seeking an experienced nurse to join our team as a Clinical Commissioning Case Manager, working across the Birmingham and Solihull area. The successful candidate will support individuals who are eligible for NHS Continuing Healthcare, requiring excellent time management, effective communication skills, and the ability to work collaboratively with health and care professionals.

This role offers an opportunity to develop skills and knowledge in a clinically led service, supporting some of the most vulnerable and complex individuals in our community. The post holder will be required to attend Multi-Disciplinary Meetings, Acute Care Facilities, individuals' homes, and care provider settings.

  • Undertake regular reviews of commissioned care, PHB, and ongoing case management.
  • Analysis of supporting documentation to establish the critical history of patients' relevant needs.
  • Attend meetings related to patients, impacting care provision and costs.
  • Ensure each patient has a care plan addressing all identified needs.
  • Negotiate care package costs and monitor packages of fully funded care.
  • Review care packages to an agreed timescale, identified by senior clinicians.
  • Identify and process safeguarding and quality issues, taking appropriate action to protect individuals' welfare.
  • Chair multi-disciplinary meetings, including case conferences and discharge meetings.

Job Responsibilities

The post holder will provide high-quality professional advice, guidance, and expertise to members of the AACHC, ICB, and wider health and social care economy. They will autonomously manage a caseload of individuals with highly complex health needs, providing significant clinical guidance to providers.
