Public Health Nurse

We are seeking a highly motivated and experienced Public Health Nurse to join our team at Homerton Healthcare NHS Foundation Trust. As a Public Health Nurse, you will play a key part in the development and safe delivery of 0-5 health visiting service, utilising best available evidence to improve the health and wellbeing of children and young people especially those from disadvantaged groups.
The successful candidate will have a keen interest in public health/community work and a commitment to undertake the SCPHN training. You will be working with our teams in City and Hackney to deliver core and targeted services to children aged 0-5 and their families.
Main Responsibilities- Support the delivery of the Healthy Child Programme within a progressive universal model of service delivery dependent on assessed needs.
- Work in partnership with the Multi-Disciplinary Team, and other agencies, to ensure that excellent standards of care are provided.
- Carry out specific nursing care for patients, families/ carers.
