Job Summary

In this challenging and rewarding Physiotherapist role, you will be part of a dynamic Integrated Community Team at Gloucestershire Health and Care NHS Foundation Trust. The successful candidate will provide assessment, goal planning, and reviews to enable patients to gain their maximum level of independence in their own homes.

About the Role

This is an exciting opportunity for a qualified Physiotherapist to join our team in Cirencester. As a key member of the Home First service, you will work closely with Occupational Therapists, Nurses, Reablement workers, and rehabilitation assistants to deliver community rehabilitation and treatment to patients with complex clinical presentations.

The caseload will be varied, ranging from rapid assessments to prevent hospital admission, Home First assessments to establish pathways of care following hospital admission, through to setting goals as part of a reablement pathway.

Key Responsibilities
  • Provide assessment, goal planning, and reviews to patients in their own homes
  • Work collaboratively with multidisciplinary teams to deliver community rehabilitation and treatment
  • Develop and implement personalized care plans to maximize patient independence
Requirements
  • Degree in Physiotherapy or equivalent qualification
  • HCPC registration and driving licence
  • Broad range of clinical experience across various clinical areas
  • Solid experience as a physiotherapist or ability to demonstrate relevant experience in rehabilitation
