About the Role

We are seeking a highly motivated and experienced Link Worker to join our team at Knowle West Health Park Company. As a Link Worker, you will play a key role in empowering individuals to take control of their health and wellbeing by connecting them with community-based and community-led social, practical, and emotional support.

Key Responsibilities
  • Take referrals for patients within the SWIFT PCN catchment area from GP practices, allied health, and community professionals.
  • Provide personalized support to individuals, their families, and carers to take control of their health and wellbeing, live independently, and improve their health access and outcomes.
  • Develop trusting relationships by giving people time and focus on what matters to them, taking a holistic approach based on the person's priorities and the wider determinants of health.
  • Co-produce a simple personalized care and support plan to improve health and wellbeing, introducing or reconnecting people to appropriate community groups and statutory services.
  • Work with a diverse range of people and communities to draw on and increase the strengths and capacities of local communities, enabling local voluntary organizations and community groups to receive social prescribing referrals.
  • Collaborate with all local diverse partners to contribute towards supporting the local VCSE organizations and community groups to become sustainable and that community assets are nurtured.
Requirements
  • NVQ Level 3, Advanced level, or equivalent qualifications or working towards.
  • Demonstrable commitment to professional and personal development.
  • Experience of working directly in a community development context, adult health and social care, learning support, or public health/health improvement.
  • Experience of supporting people, their families, and carers in a related role.
  • Experience of supporting people with their mental health.
  • Experience of working with the VCSE sector, including with volunteers and small community groups.
  • Knowledge of the personalized care approach and the wider determinants of health.
  • Understanding of equality, diversity, and inclusion.
